Physician's Information Notepad - "Memory Loss...Now What"- This project was funded, in part, through a LifeCircle Alliances' Haywood Incentive Fund grant.  The notepad was the result of the work of the Senior Services Committee of the Fairfax County Long Term Care Coordinating Council.

The Administration on Aging (aoa.gov) has Emergency Readiness Lists and publications for older adults and caregivers.  The federal agency advises updating records every two to three months.
