Chesterbrook Residences
$2500 - Award Match to Come
To provide reliable internet accessibility to the residents of Chesterbrook, an affordable (and subsidized) assisted living facility. It will support and strengthen services to older adults, their caregivers and their relatives who visit the facility. It will also allow residents easier access to mobility and transportation services that can be contacted online. Residents will also use the service to sharpen their computer skills via the free volunteer-led classes, to read hometown newspapers, to read email, and to play on-line games.
Jewish Foundation for Group Homes -MOST expansion into Fairfax County

MOST is an innovative residential one year transitional program that assists young adults (21-45) with disabilities including autism spectrum disorder, intellectual disabilities, mental illness, cerebral palsy, blindness and deafness. MOST offers each resident comprehensive Individualized Transition Plan (ITP) highlighting goals in three domains: self-sufficiency e.g. personal hygiene, laundry, self advocacy; community e.g. riding public transportation, fitness programs, arts programs; and vocation e.g. education, vocational training, and day habilitation. MOST will provide skill support; assessment and feedback to help each resident and family determine on-going needs. This grant will asset with the rental, lease or purchase of equipment.
Dementia Subcommittee, Senior Services Committee, Long Term Care Coordinating Council (Fiscal Agent: Alzheimer’s Family Day Center)

This project was developed by the Dementia Committee of the Senior Services Committee as part of Fairfax County’s Long Term Care Coordinating Council. Physician education is a primary issue in dementia care. The committee designed a convenient tear-off notepad that provides resources for family members caring for someone with dementia. The committee plans to have 500 pads printed and distributed to local physicians. Each pad has 50 pages and cost $4.50. The pilot project will begin with 100 physicians. After six weeks, the physicians’ comments will be collected and refinements will be made. Then 500 additional notepads will be printed. This grant will cover the printing costs for 334 notepads.
Shepherd Center of Oakton-Vienna

This grant will assist with operating costs. Shepherd Center provides free services to adults age 50 and older residing in the local community. Services are provided by volunteers and include: medical transport; rides to the grocery, pharmacy, bank and personal appointments; friendly callers and visitors; health and wellness counseling and education; adventures in learning series, Lunch n ‘Life; trips; and Caregivers Support Group.
