Volunteer Caregivers

LifeCircle's annual Kudos for Community Caregivers Awards-Each year five Fairfax County residents/organizations/corporations are chosen through a selection process to be recognized for their outstanding service in the care of others.

The following organizations have received funding from LifeCircle AllianceSM to support their programs:

Chesterbrook Residences

$2500   To provide reliable internet accessibility to the residents of Chesterbrook, an affordable (and subsidized) assisted living facility.  

Jewish Foundation for Group Homes -MOST expansion into Fairfax County

$5000  The LifeCircle grant will assist with the rental, lease or purchase of equipment. MOST is an innovative residential one year transitional program that assists young adults (21-45) with disabilities including autism spectrum disorder, intellectual disabilities, mental illness, cerebral palsy, blindness and deafness.  MOST offers each resident comprehensive Individualized Transition Plan (ITP) highlighting goals in three domains: self-sufficiency e.g. personal hygiene, laundry, self advocacy; community e.g. riding public transportation, fitness programs, arts programs; and vocation e.g. education, vocational training, and day habilitation.  MOST will provide skill support; assessment and feedback to help each resident and family determine on-going needs.

 

 

Dementia Subcommittee, Senior Services Committee, Long Term Care Coordinating Council (Fiscal Agent: Alzheimer’s Family Day Center)

 

$1500  This LifeCircle grant will cover the printing costs for 334 notepads. This project was developed by the Dementia Committee of the Senior Services Committee as part of Fairfax County’s Long Term Care Coordinating Council. Physician education is a primary issue in dementia care. The committee designed a convenient tear-off notepad that provides resources for family members caring for someone with dementia. 

 

Shepherd Center of Oakton-Vienna

$1000 -  This grant will assist with operating costs. Shepherd Center provides free services for adults age 50 and older residing in the local community in need of medical transport; rides to the grocery, pharmacy, bank and personal appointments; friendly callers and visitors; health and wellness counseling and education; adventures in learning series, Lunch n ‘Life; trips; and Caregivers Support Group. 


Spring Grants 2010 Selectees:

Pilot Program for Young Adults with Autism Spectrum Disorder (ASDs)

$5000 - Young Adults with Disabilities

Funding is being sought to implement a pilot program, which will provide behavior management and long-term services for adults with autism spectrum disorders(ASDs) In social/leisure settings. adult day program will address the current and anticipated gaps in services for young adults with ASDs after they complete their high school experience. The project has been designed to enhance development of social and leisure skills , introduce new leisure experiences, and identify community resources in order to provide skills to live, work and participate in recreation and leisure within the community. 

Physical Therapy Program, Alzheimer’s Family Day Center

$5000 - Older Adults

An on-going physical therapy program that will provide assessment and targeted exercise programs with the goal of keeping participants mobile and in their homes as long as possible Individual and group exercises will be led by a licensed physical therapist and will not be dependent upon Medicare approval. At AFDC, the physical therapy will be a standard part of the day center services to all participants.”

 

Multicultural Certified Nurse Aide Program 

$2000 - Workforce

To support tuition scholarships for those who can not afford their tuition for the Multicultural Certified Nurse Aide Program. Cost: $850 per student, one-third the cost of training at NO VA Community College.

Fairfax Adult Day Care Health Care Associates – to purchase Smart Board technology, software and training for one site.

 

 

 

The Arc of Northern Virginia – to develop and establish a web-based social calendar for individuals with developmental disabilities.

Disability Resource Mapping – ARC's service provider's directory populates disAbilityNavigator and enabled the Arc of Northern Virginia to leverage additional federal grant funding to further expand and sustain this disability-based directory.

PRS, Inc. – to reinstate their Dental Care Access Program for adults with mental disabilities.

Project Independence – PRS, Inc. collaborated with RPJ Housing, another nonprofit, to provide transition services and on-going support to enable five individuals with serious mental illness to move from the homes of their older adult parents and re-locate to a newly renovated home where they can live as independently as possible.

Autism Society of America, Northern Virginia Chapter – for the "Employ Me - Overcoming Obstacles" project, which provides support and promotes employment of adults with autism.

SeniorNavigator – disAbilityNavigator Website – creation of a centralized website providing individuals with disabilities, their families and professionals with a "one stop site" containing a full array of comprehensive resources.

Korean Central Senior Center – Multi-Ethnic Health Access – a workforce development program that expanded a comprehensive Personal Care Aide (PCA) course that will train 360 individuals for employment to serve 320 adults within multi-ethnic communities including Koreans, Latinos, Muslims and others.

SPecially Adapted Resources Clubs (SPARC) – with the support of the LTCCC, this program launched in 2006 and is now serving 14 young adults ages 22 to 36. SPARC provides day services to individuals with severe physical and developmental disabilities that have no other day support and would otherwise be isolated at home or without appropriate services.
